Description

Essential Duties:

  • Coordinates health education and services with physicians, staff, and other healthcare providers.
  • Provides group or individual patient education and information to manage chronic health conditions.
  • Participates in the planning, implementation and evaluation of diabetes self-management education.
  • Notifies supervisor or physician of patient’s reaction to drugs, treatments, and significant incidents.
  • Initiates assessment and education according to the individualized needs of the patient, as prescribed by physicians and/ or policy including patient and family instruction.
  • Prepares and disseminates educational and informational materials.
  • Participates in the development of programs designed to enhance health education.
  • Markets services of health education department and/or clinic.
  • Participates in multidisciplinary team meetings.
  • Maintains educational resources.
  • Performs other duties as assigned.

Education/Experience:

Bachelor’s degree in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition or related major; and completion of coordinated program or dietetic internship of not less than 900 hours under the supervision of a Registered Dietitian.  Experience in chronic disease management programs preferred.

 

Language ability: 

Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Fluent in English required; Spanish bilingual preferred.

 

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in mathematical or diagram form and deal with several abstract and concrete variables.

 

Certificates and Licenses:

Licensed and Registered, or registration eligible, Dietitian in the state of Texas. Certified Diabetes Educator preferred. Or must be certified as Diabetes Educator within two years of employment.

 

Computer Skills: 

Demonstrates working knowledge of the Internet, Outlook, PowerPoint, Word, Excel, and electronic health records software.

Other:

Ability to drive and have access to a car; maintain valid driver’s license and auto liability insurance.

While performing the duties of this job, the team member is exposed to a medical office environment having direct contact with patients and clients. Position necessitates bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver at least 25 pounds.
